Pop Warner Registration Fields Help Page.  This applies to both football and cheer, though some of the fields may only apply to either Football or Cheer (or additional information for Football or Cheer will be listed in the information below).



Field Information for above section:
  • Most of this should fill in automatically, unless you added a new child.
  • If the athlete had a previous photo on their profile, it will automatically load.  If this photo is old, it is recommended that you upload a new headshot (must be a headshot - not a full body picture - you will be able to crop when uploading.
  • If your athlete played Pop Warner previously and your SCRA account successfully linked to the Pop Warner profile, then their Birth Certificate should so complete.  Other you will need to upload a birth certificate.
    • Note - uploading the birth certificate now is not required to complete registration.  But it will be required at some point before the season begins.


Field Information for above section:
  • Family Roll (Parent 1) - this is required by Pop Warner - choose from the drop down.
  • Parent 1 information will not be listed, as that will come from the account Profile. 
  • The remaining fields are for Parent 2 and to add them to the player's Pop Warner record.


Field Information for above section:
  • Medical Doctor Signature - NOT REQUIRED, nothing needed here for registration process
  • Player Contract - NOT REQUIRED, nothing needed here for registration process
  • Report Card
    •  First, as registration has opened during the school year, reports aren't available yet and usually are not available until June.
    • Reports cards must be presented or uploaded before the 2025 season in order to be eligible to participate.  A staff member can do a visual verification of the report card.
    • More information on report cards and the Pop Warner All-American scholar program is below.


Field Information for above section:
  • Secondary Contact section is used for an emergency contact.
  • Medical Insurance information is required.
  • Family Doctor information is required.
  • There are a few required questions around medical conditions/etc as well as last concussion if applicable.
  • The athletes weight is required to complete the medical information.



Field Information for above section:
  • GPA Score - enter an estimated GPA based on 100%.  If the school year is not over, just guess.
  • The GPA Score and the Fall 2025 Grade the athlete will be in will help determine if your athlete may be eligible for the Pop Warner All-America Scholar program (both cheer and football).
  • The 2025 Medical History and 2025 Participant Contract and Parental Consent form are the final reading points of that form and to provide a digital signature.
  • For those that player Pop Warner last year, this will provide a dramatic improvement in the player forms process for the 2025 season.



Field Information for above section:
  • Is Participant New or Returning - did they participate in Pop Warner for South Charlotte last season.
  • Years of Experience - how many years of cheer / tackle football do they have
  • Jersey Size / Short Size - choose from drop down
    • Cheer will add additional size questions (shoes, skirt, etc)
  • Coach Request / Special Requests Comments - these are not required to be filled in.  But Pop Warner requires these are asked.  Any coach requests can not be guaranteed - even for returning players.  Most special requests can not be fulfilled as well.



Field Information for above section:
  • This section has all the final waivers and acknowledgements.
  • Football Commitment Understanding - this is to understand that this is not a "rec" sport, it is highly competitive and a high level of commitment is required by your athlete.
  • The remaining 4 items are the standard South Charlotte waivers.
  • All of these are digitally acknowledged and signed.
